If you have a Medicare Advantage plan through HealthPartners, your plan includes prescription drug coverage ( Medicare Part D ). Part D plans include a formulary, which is a list of drugs covered by the plan.

Most HealthPartners Medicare plans include in-network coverage for up to nine consecutive months of the year when you travel inside the U.S. but outside your plan’s service area.

Your EOC explains what the plan covers, how much you pay and more. It’s a legal document that provides complete information about how your Medicare plan works.
Your ANOC describes changes to your health plan's coverage, costs or administration for the upcoming year. These changes take effect in January.
The Summary of Benefits outlines what each plan covers and what you’ll pay for those services. The booklet doesn’t list everything that is covered by your plan, or every limitation or exclusion. For a full list of covered services, see the Evidence of Coverage .
