Life Care Planning is an important task for all of us, whether young or old, healthy or facing challenges. Through increased awareness and access to information, Arizonans of all ages can make their choices known about who will manage their medical affairs in the event of an emergency.
The Attorney General offers the forms, at no cost to you, as follows:
Complete LCP Packet (with orange DNR) - print the DNR in color in order for it to be valid
Paquete LCP completo (con DNR naranja) - Imprima el DNR en color naranja para que sea válido
Complete LCP Packet (with white DNR) - print the DNR form on orange paper in order for it to be valid
Paquete LCP completo (con DNR blanco) - Imprima el formulario DNR en papel naranja para que sea válido
Pre-Hospital Medical Directive (Do Not Resuscitate) - En Español - Orange background, print this form in color in order for it to be valid
Pre-Hospital Medical Directive (Do Not Resuscitate) - En Español - White background, print this form on orange paper in order for it to be valid

POLST Form - This form is for when you become seriously ill or frail and toward the end of life.
